Your initial appointment will consist of an evaluation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the evaluation. A complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Typically, the majority of surgical procedures will be performed at a second appointment.

Please assist us by providing the following information at the time of your evaluation:

• Your surgical referral slip and any x-rays if applicable.
• A list of medications you are presently taking. Please alert the office if you have a medical condition that may be of concern prior to surgery.
• If you have medical or dental insurance, please call us with the necessary information to verify your policy ahead of time (ie: insurance card or bring your insurance card to your visit), to allow us to help you in processing any claims.
• A valid photo ID.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the evaluation and surgical visit.

PREOPERATIVE INSTRUCTIONS

A preoperative evaluation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Patients will need to refrain from having food for six hours as well as liquids for three hours prior to surgery. A ride must remain in the office throughout the entire procedure and be able to review post operative instructions with the doctor and surgical assistant. A driver will also need to take the patient home once they have been discharged.

X-RAYS

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
